A prisoner was left unresponsive in his cell for 15 hours before he died. Staff at HMP Garth near Leyland agreed to observe Marlin Burrows, mistakenly believing he had ingested a psychoactive substance.

They only discovered he wasn't breathing when they decided to check on him after 15 hours. Paramedics were called and staff began CPR efforts, but he was later pronounced dead.

His temperature dangerously high at of 41.1 C. Following Mr Burrows' death on on August 16, 2022, the Prisons and Probation Ombudsman initiated an investigation.

A report reveals the 45-year-old received a 10-year sentence in September 2018 for inflicting grievous bodily harm with intent against his partner.
