A domestic violence risk assessment completed by police officers just weeks before Regan Tierney was stabbed to death was ‘incorrect’ and ‘incomplete’, an inquest has heard.
Officers failed to ask Regan a number of mandatory questions for a domestic violence call, as well as marking incorrect answers to others, jurors at Bolton Coroners Court were told.
Regan’s case was later given the lowest risk mark on the risk assessment scale. Regan, 27, was found dead at her home in Walkden, Salford by her dad on June 5, 2019.
She had been stabbed by her ex-partner Daniel Patten, 31, who was also found with critical injuries. He later died in hospital, and a coroner ruled his death was suicide.
