The cause of death of former child star Adam Rich has been revealed.The 54-year-old was found dead inside his LA home, more than five months ago in January. A report from Los Angeles Medical Examiner's office, has now confirmed that Rich died due to "effects of fentanyl." The report concluded that the manner of his dead was an "accident" on January 8.
