A Forest Bank prisoner who was prescribed a 'combination of drugs' died after 'serious failings' in his care, a jury have found.
Michael McDonagh, 27, was found unresponsive in his cell at HMP Forest Bank in February 2019. Over a nine-day inquest, jurors at Bolton Coroners Court heard how Michael was prescribed a number of drugs that acted in combination to depress his central nervous system - including methadone.
On Friday (September 8), a jury found the medics responsible for the prescription of methadone 'failed to carry out adequate enquiries' into Mr McDonagh's tolerance to opiate medication' before deciding to prescribe the drug.
They also found they then 'failed to monitor him in the days that followed in line with prison and national health guidelines' - a failing they concluded probably contributed to Michael’s death. During the inquest, the court heard how Mr McDonagh had been prescribed methadone just days before his death, in order to deal with withdrawal symptoms after admitting to taking the opioid, buprenorphine.
