The 32-year-old has been undergoing radiotherapy and chemotherapy to treat his inoperable brain tumour after his stage four cancer diagnosis earlier this year.
He praised his "warrior" wife Kelsey Hardwick who has been supporting him and looking after their children Aurelia and Bodhi.
In a touching message on Instagram, Tom wrote: "REALITY: Two days after this picture was taken, I was told I had a stage 4 brain tumour.
